These SMS Terms & Conditions (“SMS Terms“) govern your enrollment in and use of the text messaging program (the “Program“) offered by Linden Dental Associates LDA, LLC (“Linden Dental,” “we,” “us,” or “our“), available through our website at www.lindendentalassociates.com. By opting in to the Program, you agree to these SMS Terms, our Privacy Policy, and our Notice of Privacy Practices.

1. Program Description

The Program sends SMS/text messages from Linden Dental to patients and prospective patients who have opted in. Messages may include:

  • Appointment reminders, confirmations, and recall notices;
  • Treatment-related follow-up and post-operative instructions;
  • Responses to inquiries you submit;
  • Billing notifications and payment reminders;
  • Practice updates (for example, office closures, schedule changes, or weather-related notices); and
  • Occasional informational or promotional communications about our services.

Messages are sent from Linden Dental through our SMS service provider, Twilio Inc.

2. How to Opt In

You can enroll in the Program by:

  • Entering your mobile telephone number and checking the SMS consent checkbox on our appointment request or contact form at www.lindendentalassociates.com;
  • Providing written consent on an in-office patient registration or update form;
  • Verbally authorizing a Linden Dental team member to enroll you in the Program, where permitted; or
  • Replying START or YES to an invitation text we send you.

By opting in, you consent to receive recurring automated text messages from Linden Dental at the mobile number you provided, sent using an automatic telephone dialing system or equivalent technology. Consent is not a condition of purchase or of receiving dental services.

3. Message Frequency

Message frequency varies based on your appointment activity, treatment plan, and the preferences you have selected. You may receive up to approximately 10 messages per month. Message frequency may be higher during active treatment or in the period immediately before a scheduled appointment.

4. Message and Data Rates

Message and data rates may apply. Your wireless carrier may charge you for each message sent and received. Check with your mobile carrier for details about your messaging plan. Linden Dental is not responsible for any charges from your carrier.

5. How to Opt Out (STOP)

You can cancel the Program at any time by texting STOP to any message you receive from us. After you send STOP, we will send you a confirmation message, and no further messages will be sent, except that we may send a single confirmation of your opt-out. If you want to rejoin the Program, you can sign up again using any of the opt-in methods described in Section 2.

Opting out of SMS communications will not affect your ability to receive care at Linden Dental, but it may limit our ability to send appointment reminders and other important notices. You can also update your communication preferences by contacting us using the information in Section 12.

7. Supported Carriers and Carrier Liability

The Program is available on most major U.S. wireless carriers, including AT&T, Verizon Wireless, T-Mobile, US Cellular, Boost Mobile, MetroPCS, Cricket, Virgin Mobile, and others. Carriers are not liable for delayed or undelivered messages. Delivery of messages is subject to effective transmission from your wireless carrier’s network and is outside our control.

8. Eligibility

To participate in the Program, you must be at least 18 years old, or the age of majority in your jurisdiction, and the account holder or authorized user of the mobile device associated with the phone number you provide. If you are enrolling on behalf of a minor patient in your care, you represent that you are the parent or legal guardian of that minor and that you are authorized to consent to receive messages relating to the minor’s care.

9. Privacy and Data Sharing

We respect your privacy. Information you provide when enrolling in the Program is governed by our Privacy Policy and, with respect to Protected Health Information, our Notice of Privacy Practices.

No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

We share mobile information only with subcontractors acting on our behalf to operate the Program — specifically our SMS service provider, Twilio Inc., and, where applicable, our patient communication platform — and only to the extent necessary to deliver messages to you. These subcontractors are contractually prohibited from using your mobile information for their own marketing or promotional purposes.

We do not sell or rent your mobile telephone number or other personal information.

10. Your Responsibilities

You agree to:

  • Provide accurate and current information, including your correct mobile telephone number;
  • Promptly notify us if you change or deactivate your mobile telephone number so that we do not send messages intended for you to another person; and
  • Comply with these SMS Terms and all applicable laws.

You represent and warrant that you are the subscriber or authorized user of the mobile telephone number you provide, and that you have the authority to consent to receive text messages at that number.

11. Changes to These SMS Terms

We may modify these SMS Terms at any time. Material changes will be communicated by posting the updated SMS Terms at this URL and updating the “Last Updated” date above. Your continued participation in the Program after any change constitutes acceptance of the modified SMS Terms.
