For treatment fees under $1,000

- 3, 6 and 12-month plan options
- No interest if the balance is paid within the specified time period
Extended Payment Plan
For treatment fees ranging from $1,000 to $25,0000
- Monthly payments as low as $25* (for a fee of $1,000)
- 24, 36 and 48-month options
- 9.90% Interest rate
